 Dear Patient/Parent/Guardian,

To begin, I would like to thank you for the trust you have given me over the years as your ENT physician.  Taking care of you and your family has been an honor for my staff and me.  Your wellness and health are priorities to us.

I am writing today to inform you that I will be closing my practice due to relocation to another state effective as of December 31, 2011.  I will be available until this date for your care.

It is important that you continue with appropriate medical care.  There are other ENT's in Valdosta, as well as, in Thomasville, Moultrie, Tifton,  Douglas, Waycross, and Perry, FL to assist you with your continued care.

The attached page has a HIPAA authorization form to release a copy of your medical records.  Upon receipt of this signed form, my office will forward a copy of your medical records to you or the physician you delegate.

After the close of my practice, signed requests for copies of medical records can be emailed to georgiaent1@gmail.com

 If you have not selected a new physician by the time my practice closes, you can obtain acute, critical, or emergency care by calling or visiting the emergency department at Smith Northview Hospital or South Georgia Medical Center.
